GRADE 1-8 REGISTRATION
Thank you for your interest in St. Columban School. We are excited to begin the registration process with you! We celebrate over 95 years of academic excellence providing a challenging, value-centered education, and are eager to support you in your role as primary educator of your child.

Completing the registration forms is the first step in the admission process for your child. You may download the PDF forms in the top right corner or complete the online forms. Either way, please complete all these steps:
Registration Forms
Bring in your completed forms to the school office on or after 1/28/2024. Email applications will be accepted 1/29/2024.
Screening Fee
Bring a screening fee of $30 to the office (checks made payable to St. Columban)
Birth Certificate
Bring your child’s original birth certificate (a copy will be made and the original returned)
Baptismal Certificate
Bring your child’s original Baptismal Certificate (a copy will be made and the original returned). If baptized at St. Columban, the certificate is not needed.
